Grant proposals must be submitted in writing on an Evan's Life Foundation grant application form to qualify for consideration. Unfortunately we cannot consider any grant request that is not submitted on an Evan's Life Foundation grant application. Any applicant over the age of 18 does not qualify for a grant through Evan's Life Foundation. All applicants should provide as much additional or supportive information as possible. We would like in-depth details of any programs you may wish Evan's Life Foundation to sponsor and the number, age and socioeconomic status of the child(ren) in need.

Please fill out the form completely and mail it to: Evan's Life Foundation 101 South First Street Dundee, IL 60118. You may also fax or email grant applications to 847-428-6564 or pat@evanslife.org. We must receive the completed application form 5 days before the next slated board meeting in order for us to review it at that time.
